After almost 3 years, we are now able to take on a limited number of new patients. We plan to do so gradually and conscientiously so that we may continue to provide timely, quality care to our current and future patients. For this reason, we ask that all prospective new clients fill out the New Client Application form below.  By reviewing applications in advance, we can carefully manage our doctors’ caseloads, and ensure that our hospital will be a good fit for you and your pet’s needs.  Our receptionists are not able to expedite or provide any information regarding this process.   Your patience and understanding is appreciated.

We hope to call approved applicants roughly 1 to 3 weeks after receipt of your application. At the time of scheduling, we will require a NON-REFUNDABLE pre-payment of the office visit fee to secure your appointment time.
